- Bring a current medication list
- Bring photo identification and your Medicare card
- Arrive 5–10 minutes early
- Complete the new-patient registration form
Registration and records
All new patients complete a registration form at their first visit. If you want records transferred from your previous clinic, complete a medical-history request form and send it to that clinic.
Getting here
Disabled parking is available for patients on-site. Other on-site spaces are reserved for doctors and staff. Nearby street parking and free parking across the road are available.
Prescription restriction
The clinic does not provide S4 or S8 drug scripts to new patients on weekends or public holidays.
