Introduction
This policy explains how personal information, including health information, is collected and used within the practice and when it may be shared with third parties.
Consent and purpose
Registering as a patient provides consent for GPs and relevant practice staff to access and use information needed to provide healthcare. Additional consent will be sought for unrelated uses.
Information is also used for directly related activities including claims, payments, practice audits, accreditation and staff training.
Information collected
This can include names, date of birth, addresses and contact details; medical history, medications, allergies, adverse events, immunisations, social and family history and risk factors; Medicare and healthcare identifiers; and health-fund details.
Patients may deal with the practice anonymously or under a pseudonym where practical and lawful.
How information is collected
Information may be collected during registration and care, through the website, email, SMS, telephone, online booking or social media, or from a guardian, healthcare provider, health fund, Medicare or the Department of Veterans’ Affairs where direct collection is not practical.
When information may be shared
Information may be shared with involved healthcare providers and contracted service providers, when required or authorised by law, to prevent a serious safety threat, for confidential dispute resolution, statutory notification, electronic prescriptions or My Health Record.
Access is limited to people who need the information. Information is not shared outside Australia except with consent or in exceptional circumstances permitted by law, and is not used for direct marketing without express consent.
Research and population health
The practice may analyse information to improve its services and may provide secure, de-identified data for population-health improvement. Patients can ask reception not to include their information.
AI-assisted clinic notes
Doctors may use AI tools to assist with writing and managing clinical notes. The practice states that information is handled confidentially under its privacy protocols.
Storage and protection
Personal information may be stored in different forms and is stored securely by the practice.
Access and correction
